This certificate is to be completed by CIF-SS schools for each student athlete participating in the WRESTLING MASTERS MEET.  The form is formatted as a "type in".

This certificate is to be completed FOR ALL SPORTS by CIF-SS schools for each student athlete that is on the roster of a team in a CIF Southern Seciton Playoff Game.  The form is formated as a "type in" - schools will provide their own paper.

CIF-SS TICKET CONTROL SHEET

This form is a resource for schools to assist in ticketing process when hosting events.  Effective Fall 2010, it is NO longer a requirement for schools to submit this form with the Official Financial Report.

This form shall be completed whenever any incident occurs in connection with an interscholastic athletic contest.
